① Initial Account Creation
Member registration is required to use the "Eligibility Certificate."
For the initial login, you will need your insured person's symbol and number, so please have one of the following ready: My Number Portal, Notice of Eligibility Information, or your Health Insurance Card.
1. Member Registration
Access the URL sent via email and enter your email address and password.
2. Authentication
Enter the authentication code sent to your registered email address
3. Enter Number
Enter your health insurance number, surname, first name, etc.
※Please switch the display language to Japanese before entering your information.
4. Select Eligibility Certificate
※ This step may be skipped depending on your usage environment.
5. Application for Issuance
Review the Terms of Use and Privacy Policy, then submit your application
6.Two-Factor Authentication
If you use SMS authentication, enter your phone number.
If you can not use SMS authentication or if it is unavailable , you can also use an authentication app such as Google Authenticator or Microsoft Authenticator. For more details, click here.
7.Two-Factor Authentication
Enter the authentication code sent to your registered phone number.
If you are using an authentication app, the authentication code will not be sent. Please check the code displayed in your authentication app. For more details, click here.
8. Select Application Reason
Select the applicable reason for application
9. Select Application Reason (Address Change)
※ If there is a change in address information, enter the information
10. Display Eligibility Certificate
※ Valid for 48 hours.
※When presenting it at medical institutions or similar facilities, please show it in Japanese.
For smooth use from next time onwards, it is convenient to "Add to Home Screen" the Eligibility Certificate page.

②Login Method for 2nd Login and Beyond (When less than 48 hours have passed since last login)
If you have completed the initial registration and less than 48 hours have passed since your last login, you can log in again by following the steps below.
※ If cache remains on your PC or other device, some steps may be skipped.
1. Login
Enter your email address and password
2.Two-Factor Authentication
If you are using SMS authentication, enter the verification code sent to your registered phone number.
If you are using an authentication app, the authentication code will not be sent. Please check the code displayed in your authentication app. For more details, click here.
3. Select Eligibility Certificate
※ This step may be skipped depending on your usage environment.
4. Select Eligibility Certificate
Press "Display Eligibility Certificate"
5. Select Eligibility Certificate (Reapplication)
※ If you need to reapply for the eligibility certificate, press "Reapply"
6. Display Eligibility Certificate
※ Valid for 48 hours.
※When presenting it at medical institutions or similar facilities, please show it in Japanese.
③Login Method for 2nd Login and Beyond (When 48 hours or more have passed since last login)
If you have completed the initial registration and 48 hours or more have passed since your last login, you can log in again by following the steps below.
